PROGRAMS & SERVICES

Senior Outreach Services (SOS)

Description
SOS (Senior Outreach Services) is a fee–for- service senior care program which provides a safety net for relatives and friends who need help caring for their loved ones.  Skilled professionals assess the needs of vulnerable and frail clients throughout Los Angeles, and work with clients and their families to develop appropriate plans, obtain needed senior care and provide on-going follow-up and monitoring.  SOS is the Ultimate in senior care services.

S.O.S. clients receive the personalized care of the community’s most established agency in providing senior care for the elderly. Friendly and knowledgeable care managers are available to make regular visits to check on our clients’ needs and well being, monitor caregivers, respond to any changes, as necessary and coordinate all senior care.  Our services are available in most of the Greater Los Angeles area.

Services
Following a comprehensive assessment, with recommendations and referrals, our staff are available to locate, coordinate, and supervise all necessary in-home and community based senior care services, including:
• Attendants or companions who can run errands, prepare meals, do housekeeping and laundry, and provide escort to doctor visits and other appointments as well as a variety of other senior care services.
• Caregivers to assist with medications, bathing, dressing and walking.
• Transportation to wherever you need to go.
• Home visits by nurses, hairdressers, therapists and other professionals.
• Home safety: evaluations for clean- up, repairs,
arrangements for grab bars and other modifications.
Our staff has vast knowledge of placement alternatives when those needs arise.
We also provide on-going monitoring and updating to family members, consultation and counseling, and advocacy, as needed.

IN-HOME HELP - WE'RE YOUR BACK UP
Many older adults and their families feel that staying in their own home for as long as possible is the most desirable option. Achieving this goal may require putting additional support, such as a professional caregiver, in place.

KEEPING SAFE AT HOME
The risk of falls is one of the greatest dangers for older adults, and can result in injury and even death. Individuals with dementia must be given special consideration. There are many steps that can be taken ahead of time to ensure greater safety and security in the home.

MENTAL HEALTH ISSUES AND AGING
As with individuals of all ages, older adults may experience issues related to mental health. For many seniors, the stigma is more severe since most seniors grew up in an era when having a mental disorder diagnosis meant you were ÒcrazyÓ and institutionalization was likely. However, treatment can be successful, and key to sustaining quality of life.

PLANNING FOR THE FUTURE
We never know what the future holds. The more we plan ahead, the better prepared we can be. This is especially true with matters of health and aging. Talking about aging and planning for its accompanying challenges may not be easy, but it may help parents and children avoid crises later on.

RESIDENTIAL OPTIONS
