Skilled professional care managers from the JFS Concierge Care Management program assess the needs of vulnerable or frail clients of all ages--from infants to the frail elderly. This fee-for-service concierge program is designed to accommodate the unique needs of clients and their family members by developing a roadmap for care, maximizing safety, coordinating needed services and support, and providing on-going follow-up and monitoring.
JFS Private Care clients receive personalized service from Jewish Family Service, the community’s oldest and most established agency in providing skilled professional care for the elderly and disabled.
Knowledgeable, professional, compassionate care managers provide the following:
- Assess clients’ needs and safety
- Consultation and referrals
- Regular visits to check on clients’ ongoing needs and well being
- Arrange and monitor caregivers
- Respond to any changes as necessary
- Coordinate care
- Counseling and support to clients and family members
